Community Services Block Grant

PACS is funded, in part, and administers services under a contract with the Cabinet for Health and Family Services from the Community Services Block Grant (CSBG) of the U.S. Department of Health and Human Services.

The CSBG program is designed to provide services and programs that directly and indirectly address the needs of the low-income consumer. This is accomplished through our partnerships with other neighborhood-based organizations, churches, and private donors.

The Community Needs Assessment only takes a few minutes to complete, and it will assist in helping people, meeting community needs, and providing hope. All information will be kept confidential, and your name is not required on the questionnaire.
